MedSchool Forums USMLE QBank A 48-year-old woman with a history of pituitary surgery and irradiation is scheduled for elective surgery. She currently requires replacement thyroxine, hydrocortisone, estrogen, and progesterone. In the perioperative period you should treat her with which of the following?
  1. Glucose infusion
  2. Increased hydrocortisone
  3. ACTH infusion
  4. Increased estrogen
  5. Increased thyroxine
Spoiler for Answer:
Increased hydrocortisone. Sudden withdrawal of corticosteroids could be fatal if the patient could not mount a stress-response (tachycardia and cortisol-mediated increase in blood-pressure) to the surgery.

Surgery Total Knee Replacement Streaming Video and Other Surgeries

I just found a great video for a total knee replacement that is pseudo minimally invasive (8cm cut?) and you can watch it streaming video...check it out: http://www.or-live.com/methodisthealth/1337

Also...www.or-live.com has many different surgical videos for those late nights when First Aid is getting blurry and those damn systemic mycoses keep drifting from memory.

And...orthogate.com seams to be a great website for orthopedics!

WOw...awesome awesome awesome website. Unfortunately my friday night USMLE study went down the drain as I watched a "Computer Assisted Hip Replacement Surgery". So very cool how they are integrating computers and surgery. I find it intersting how differnt surgeons use different approaches...the Robert Wood Johnson Docs choose the posterior muscle splitting approach where another group (can't remember the hospital but watched an operation video last week in clinic) go for the minimally invasive anterior approach. Check out Or-live! And you can even submit email questions during surgery! There is a gastric Rou-en-Y coming up this week.
